White spot viral disease, caused White Spot Syndrome Virus have still a threat to shrimp farming in Indonesia, because it resulted in death at an early age, between one to two months, even in the late shrimp lifespan of less than one to have started developing. Disease control program were need to be perform, through surveillance approach in order to determine the prevalence and risk factors in the case of WSSV. Active surveillance was carried out in the shrimp farming centers in Lampung and Banten province. Sampling was carried out by two stage systematic sampling, by first selected farm, then selected ponds in every farm. Shrimp pond was point out as sample unit, taken randomly on pond, and purposiv for shrimp in each pond were elected.  A total of 183 samples collected from the area of South Lampung, Pandeglang and Serang. Analysis done using Statistix as software. Based on surveillance results showed that the prevalence of WSSV South Lampung 54.3%, 88.9% Serang and Pandeglang 100%. Some risk factors are significant with P <5% and very significant with P <1% is a pond fertilization (P = 0.0217), screening the seed before stocking (P = 0.0257), water disinfection (P = 0.0101) and replacement of water systems (P = 0.0001).
